REQUEST FOR PROPOSALS (RFP) - Concrete Work Village of Park Forest – South Suburban Trades Initiative Project Address: 261 Miami Street, Park Forest, IL 60466 PIN: 31-35-414-003-0000 I. Invitation for Bids The Village of Park Forest, Illinois, (“Village”) will receive Bids for concrete work at 261 Miami Street. The deadline for bids is 5:00 pm, Friday, March 13, 2026. . Bids should be delivered via email to the project manager (David Tracy) at: davidstracy13@gmail.com. Please submit bids in PDF format and a maximum size of 20 MB with the subject line of ‘Park Forest 261 Miami Concrete Bid’. All bids must include permit fees. II. Project Description The work consists of the demolition, removal, and installation of concrete structures. The scope of work involves the following locations: Driveway: 11’ x 61’ Rear “sidewalk”: 6’ x 24’ 9” – runs from corner of house by driveway to the family room wall. Seven (7) sections of damaged public sidewalk. At each location the work requirements are: Demolition and removal of existing concrete: Preparation of subgrade and installation of new concrete Compliance with all Village, County, and State regulations III. Prevailing Wage Requirements This contract is subject to prevailing wage requirements. No less than the prevailing wage as. determined by the Illinois Department of Labor shall be paid to any worker performing work under this contract. The current prevailing wage rates as determined by the Illinois Department of Labor can be found at https://www2.illinois.gov/idol/Laws-Rules/CONMED/pages/2018- rates.aspx. The current wage determination in effect at the time the contract is executed shall govern the contract. The awarded contractor will be required to submit valid documentation/certified payrolls during the course of construction certifying that the prevailing wage requirements are met. The Village of Park Forest is located in Cook and Will Counties. IV. Bid Submission Details All bids must be emailed to the Project Manager, David Tracy, at: davidstracy13@gmail.com. Any questions regarding this Invitation for Bids must be submitted in an email to David Tracy at the above email address no later than five days prior to the submission date. All Requests for Information (RFIs) answers will be resubmitted to all bidders so all potential applicants will have the same information. V. Examination of Job Site Bidders shall thoroughly examine the property in order to ensure they are fully informed of the actual conditions and requirements of the work prior to bidding. Failure to detect potential problems shall not relieve the selected Contractor of its obligations, nor shall the selected Contractor be allowed any extra compensation due to issues that arise because of its failure to be fully acquainted with the sites prior to bidding. VI. Time of Performance Time is of the essence in the completion of this project. All work must be completed, and invoices submitted to VOPF for payment no later than April10, 2026.. Activities shall be limited to Monday through Friday, from 7:00 am to 6:00 pm. VII. Insurance Requirements Contractor shall procure and maintain, for the duration of the contract, insurance against claims for injuries to persons or damages to property, which may arise from or in connection with the performance of the work. See Appendix A for detailed insurance requirements. VIII. Permits The Contractor shall obtain all required federal, state, county, and local permits prior to starting the work. IX. Award of Contract The Village reserves the right to review all bids submitted for a period of thirty (30) days after the date of submission, and by submitting a bid, the Bidder agrees that the amount specified in its bid shall remain in full force and effect from such thirty (30) day period. No Bidder shall modify, withdraw or cancel its bid or any part thereof for thirty (30) days after the bids have been opened, and no attempted modification, withdrawal, or cancellation shall be valid. The Contract shall be deemed as having been awarded when formal memorandum is provided.by the Economic Development and Planning Department. The Village reserves the right to terminate the whole or any part of this Contract, upon written notice to the Bidder, in the event that sufficient funds to complete the Contract are not available.
