The County intends to award two 1-year contracts (one for the Mid-County FRCs and one for the Oakland Central FRC) for the approximate term of August 1, 2026 – June 30, 2027 (with the option to renew annually for 4 additional years) to the Bidder(s) selected as the most responsible Bidder(s), whose response conforms to the RFP and meets the County’s requirements. Table 1 below summarizes the two award regions, school districts each lead agency will work in, and anticipated award amounts for the initial contract.

This RFP includes two award regions. A Bidder may submit a bid for one or both of the award regions. Bid responses must clearly state if the Bidder is bidding for (i) the Mid-County FRCs award only; (ii) the Oakland Central FRC award only, or (iii) both. If bidding for both awards, Bidder must follow the detailed instructions included in Exhibit A—Bid Response Packet, Required Documentation and Submittals below, and include separate, clearly labeled submittals where required.

Responses to this solicitation must be complete. Responses must address all the requirements identified within this solicitation and all related documents, including any Addenda. Failure to meet the Bidder Minimum Qualifications may also be considered an incomplete response and may result in the disqualification of the Bidder.
Bidders, its principal, and named subcontractors are not identified on the list of Federally debarred, suspended, or other excluded parties located at www.sam.gov/SAM.
Proposals will be evaluated considering the RFP specifications and the questions below:
1. How well has the Bidder demonstrated an understanding of the need for services that support increased family health access and improved family wellness?
2. How well has the Bidder demonstrated an understanding of the unique needs of residents in the communities seved by the FRCs?
3. How well has the Bidder demonstrated an understanding of potential challenges in program implementation and how to address them?
Proposals will be evaluated, including considering the RFP specifications and the questions below:
1. To what extent does the Bidder have the desired experience the scope of work outlined in the RFP?
2. How well does the Bidder describe an example project demonstrating the desired relevant experience?
3. How well does the Bidder demonstrate knowledge of public benefit programs available in the State of California, including eligibility criteria?
4. How well does the Bidder demonstrate familiarity of health and wellness resources in communities served by the FRC Initiative?
5. To what extent does the Bidder have experience working with the communities to be served?
6. To what extent does the Bidder describe any unique resources, strengths or approaches that make the Bidder uniquely responsive to meeting the requirements of this RFP?
Proposals will be evaluated considering the RFP specifications and the questions below:
1. How well does the Bidder’s proposed services align with FRC Initiative’s allowable services, goals and desired outcomes?
2. How well does the Bidder describe their approach to coordination of care?
3. How well does the Bidder describe plans to engage school district staff in planning and implementation?
4. How well does the Bidder describe their plans for partnering with community-based organizations and public agencies?
5. How well does the Bidder detail existing data collection infrastructure, and demonstrate the ability to provide required data to the County, including data on unduplicated individuals served?
The points for Cost will be computed by dividing the amount of the lowest responsive and responsible bid received by each Bidder’s total proposed cost.
