    Description

    Vermont has received a Rural Health Transformation (RHT) grant of $195 million in Year One of a five-year opportunity. The purpose of the grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost.The purpose of this Notice of Funding Opportunity (NOFO) is to provide funds to primary care and/or specialist healthcare providers to purchase or lease electronic consultation (e-consult) technology in support of workforce capacity building. These funds will support the implementation of e-consult platforms to ensure an interoperable means of access for primary care providers to consult with specialists. Grants will be provided to providers to enable purchase of technologies to support the build and function of an e-consult platform. Funding may be used for technology upgrades as well as startup program coordination costs that do not have an alternative funding source. Access to e-consult services will save patients from having to travel to separate appointments and help strengthen local primary care capacity.
