    Evaluation Criteria

    • Program Abstract (15 pts)
      1. Describe your proposed program and how it is going to meet the needs described in the RFP. This should be an overview.
      2. Describe your proposed staffing structure including the roles and reporting responsibilities that meet the needs and expectations listed in the RFP.  Include how your agency will meet the clinical supervision requirements of DHS 34.21(7).  How will the clinical supervisor be integrated into the program structure? How will they be kept informed of client needs/changes and continued eligibility for MA Crisis?
      3. Please upload position descriptions for all key roles proposed to deliver services under this contract. Position descriptions must include minimum education, experience, and any required credentials/certifications. These may be separate attachments uploaded in the vendor questionnaire.
      4. Describe how your program will track service usage. How will your agency use this to achieve the goals in the RFP?
    • Program Strategies and Activities (30 pts)
      1. Describe specific strategies and activities that your agency will use to achieve the stated objectives, expectations and desired outcomes of the RFP.
      2. Describe strategies and activities that will be used to make this program inclusive, culturally responsive and accessible to diverse populations.
      3. Describe the services that your agency proposes to provide and how you will target different service options to best fit resident need.  Describe how you will adjust your approach to service provision as individual needs/levels of engagement change. How will you work to connect residents to the most comprehensive long-term service options available? What strategies will your use to engage residents actively in supportive services?
      4. Describe the way in which your program will define roles, reporting responsibilities, and collaboration with property management, the County and the City of Madison. If staff will be spread across multiple service categories (MA Crisis, MA TCM, MA CCS), describe the division of labor and separation of duties across categories. If including peers in your staffing model, describe their role within the program.
      5. Describe how you will foster a sense of community within the resident population and promote positive engagement between the resident population and the neighborhood.
      6. Describe how your program will limit emergency calls to 911, emergency rooms and overall police presence on the property.
      7. Describe how your programming will limit problematic use of substance use on the property. How will your program target interventions according to each individual's readiness for change?
      8. Describe how you will develop and maintain a resident advisory committee and ensure that this committee has input into policy, changes, recommendations and considerations.
    • Experience and Qualifications (20 pts)
      1. Include your agency’s mission statement and discuss how this proposed program aligns with that mission.
      2. Describe the experience and qualifications of your agency to provide programs that are welcoming to persons of all backgrounds and cultures including any explicit plans your agency is undertaking to improve in this area.
      3. Describe the experience and qualifications of your agency to be the primary case management service provider, including your experience in providing trauma informed care, harm reduction interventions, working with chronically homeless persons, and providing MA billed services.  Include your philosophy for serving individuals experiencing chronic homelessness.
      4. Include staff turnover calculation for the previous year. If you had 20% or more turnover in a certain staff position/category, please explain. Also address any noteworthy staff retention issues or policies to reduce staff turnover.
      5. Describe your agency’s experience in collaborating with various service providers and stakeholders. Who are the stakeholders and what is the role that they play in decision making?
      6. Submit 2 letters of support. Letters should be specific to the service offered. Letters should be from agencies you have done business with or those with whom you have collaborated. These may be separate attachments.      
    • Quality Improvement (15 pts)
      1. Describe any programmatic or administrative improvements that have improved your agency’s ability to deliver services. Also describe any ongoing quality assurance procedures and practice your agency has in place.
      2. Describe how program data is or will be used to drive quality improvement efforts, including the measurement of and performance on client outcomes. Describe your agency’s efforts to improve service qualities and outcomes for the proposed program.
      3. Describe the process for obtaining client feedback, including how client feedback will be used to inform program quality improvement. Be specific when describing how the feedback will be obtained including the method, frequency and process for review.
    • Cost (20 pts)

      Points for cost will be awarded using a relative formula. The proposal with the lowest cost will receive the maximum number of points available for this section. All other proposals will receive a proportion of the cost points according to the following calculation:

      In plain language: the lowest-cost proposal earns all available points. Other proposals earn fewer points based on how their cost compares to the lowest. For example, if your proposal costs twice as much as the lowest proposal, you would receive about half of the cost points.

    • Local Vendor Preference (5 pts)

      Per Dane County ordinance, a local Dane County vendor automatically receives five (5) points toward the evaluation score.

      Vendors located within the counties adjacent to Dane County (Columbia, Dodge, Green, Iowa, Jefferson, Rock, or Sauk) automatically receive two (2) points toward the evaluation score.

      Locally Operated Vendor means a supplier or provider of equipment, materials, supplies, or services which has an established place of business within Dane County and whose business is registered and authorized to do business in the State of Wisconsin. An established place of business means a physical office, plant, or other facility. A post office box address does not qualify a vendor as a Locally Operated Vendor. Dane County Ordinance 25.04(5)
