    Description

    THP-Plus is a transitional housing program for young adults who exited foster care, including those supervised by Probation, on or after their eighteenth (18th) birthday and are not yet twenty-five (25) years of age. Former foster youth can remain in the THP Plus services for up to three (3) years.THP-Plus services are being sought with a focus on assisting former foster youth to move from dependency to self-sufficiency by providing youth with safe stable housing, supportive service, case management, housing services, education, and employment services. These services help prepare the youth participant for the workforce and/or includes academics as a pathway to earning a high school diploma or high school equivalency certificate, vocational or other degrees through higher education.The housing models provide youth with two (2) models: The Host Family Model and the Scattered Site Model. In the Host Family Model, the youth is housed with a former foster parent or a support person with whom they had connection. In the Scattered Site Model, the youth live independently in an apartment located in an area near their school and/or their employment. Housing models include apartments, single-family dwellings, condominiums, college dormitories, and host family models. Shelters or temporary accommodations with friends are not considered a housing model.The former foster youth participating in the THP-Plus can receive cash assistance as stated in the Scope of Work, as funding applies. The goal of THP-Plus is to provide former foster youth with secure and stable housing after graduating from the program and to progress the youths’ life goals in the area of education, physical/mental well-being, connections to the community, employment, and self-sufficiency.The successful Proposer must meet all the requirements and expectations set forth herein, as well as all the general contractual and insurance requirements mandated by the County (“Terms and Conditions). Terms and conditions may be modified through the RFP process. Proposers must thoroughly explain how their Proposal meets or exceeds the service and deliverable expectations specified in the RFP.Award may be made to the Proposer whose Proposal provides the greatest overall benefit to the County, taking into consideration: a) the Proposer’s conformance to the requirements in the RFP; b) the Proposer’s qualifications and ability to perform the service required within the specified time; c) the quality of performance in previous contracts and references; d) financial ability to perform the contract; e) the ranking factors, and f) pricing. The County reserves the right to cancel the solicitation and to reject any or all Proposals in whole or in part and is not bound to accept any Proposals if rejection of that Proposal is determined to be contrary to the best interest of the County.This RFP contains instructions governing the requirements for Proposals, including the format in which Proposal information is to be submitted and the material to be included, the requirements that must be met to be eligible for consideration and the responsibilities of the Proposer before and after implementation.
