    Description

    Van Contractor Requirements (revised 6/5/19) Vehicles other than school buses originally designed and constructed at the factory for nine passengers or less, not including the driver, must comply with the following Federal and State Ohio Department of Education and Workforce (ODEW) Rules and Regulations: 1. The vehicles shall be equipped with safety equipment as follows: fire extinguisher, first aid kit, body fluid cleanup kit, fuses, spare fuses, and emergency reflectors 2. The vehicle shall have a rooftop sign marked “School Transportation” 3. The name of the contractor shall be clearly marked on the side of the vehicle. 4. These vehicles shall be inspected by a qualified mechanic no less than two times per year. This inspection shall cover at the minimum all applicable sections of the school bus inspection detailed in paragraph (c) of rule 3301-83-11 of the administrative code. In addition, periodic maintenance intervals must be established and implemented. Documentation and proof of these inspections and service procedures, in addition to all vehicle records required under rule 3301-83-14 of the administrative code, shall be kept on file within the contractor’s office and available for viewing by the Director of Transportation or designee. 5. It is the responsibility of each vehicle driver to complete and document a daily pre-trip inspection. Follow the Pre-Trip Instructions and Form as printed on pages 13-14 of the Ohio Preservice Driver Training Program Ohio School Van Driver Manual dated April 2015 6. Loading of these vehicles shall be in compliance with the passenger, weight, and other associated restrictions as identified by the original equipment manufacturer. 7. Van drivers must have the following documents completed per State and Federal requirements and on file (copies must be provided to the Columbus City Schools Transportation Department Compliance Officer): a. A written job application b. Employment References c. A pre-employment D.O.T. drug test d. A FMCSA report from their previous employer if they were subject to D.O.T. required drug/alcohol testing (forms are available on ODEW website) e. Copy of a valid driver’s license f. A T8 physical g. A TB test is required by Columbus City Schools if the applicant was out of the USA for one year or more or was foreign born. h. A BCII State background check i. A BCII Federal (FBI) background check j. An initial and semi-annual (monthly) motor vehicle records check obtained through ODEW web reporting k. Proof that data has been reported to the ODE web reporting system and the Columbus City Schools Transportation Department must have authorization to be a secondary viewer of this data l. ODE Van Pre-service Certificate and proof of attendance at an annual 2-hour van drivers’ in-service m. 21 years of age (required by Columbus City Schools) and 2 years driving experience n. Physically capable of lifting and managing preschool and special needs children when necessary (proof must be documented) o. The ability to cope with stressful situations (proof must be documented) In addition, Columbus City Schools Transportation Department requires the following: 1. All vehicles and documents will be subject to spot inspections both on the road and at the contractor’s base of operations 2. All lifts must be approved by the Columbus City Schools Transportation Department for use and all drivers must be trained in the proper and safe operation of these lifts 3. All wheelchair tie-downs shall be forward facing 4. Must demonstrate proof of ability to use the ODE web based reporting system to the Columbus City Schools Operations Supervisor. 5. Must show proof of compliance to the DOT Drug and Alcohol Testing program which requires the following: a. Use of a DOT approved drug/alcohol test provider b. Quarterly reports from test provider showing specific tests given and results c. Monthly random pool list d. Pre-employment drug test e. Random Drug and (or) Alcohol testing f. Reasonable suspicion drug and alcohol testing g. Post-accident drug and alcohol testing h. Return to duty (after 30 or more days absent for any reason) drug and alcohol testing – there is no return to duty for drug or alcohol use rehabilitation; employees are terminated if they have a positive test result i. Copy of the company’s Drug and Alcohol Testing Policy 6. All accidents must be reported to the Columbus City Schools Transportation Safety Supervisor by telephoning 365- 7659 or 365-5638 (after hours – the safety supervisor will direct you to fill out the appropriate ODE and OHP forms) --- QUESTIONS AND ANSWERS --- Q: Please define "highly fragile students".
