Information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide the services required in the Statement of work. The firm must include all certifications, qualifications, and training certificates to perform work on the outlined in the Performance Work Statement. Synopsis 1.0 OBJECTIVES This Firm Fixed Priced contract will be a base plus (4) option years for Neuromodulation therapy device and accessories. The awarded contractor will provide the CPU with devices monthly on a scheduled basis without delay or disruption. 2.0 REQUIREMENTS/SALIENT CHARACTERISTICS The CEFALY Connected Neuromodulation therapy device has the minimum characteristics as described below: Physical Characteristics Device External trigeminal nerve stimulation (eTNS) device Electrodes 3 pack Battery 7-hour battery life Bluetooth Compatibility to connect to cell phone Power 110v Charging dock and USB Cable Treatment Parameters Acute Prevent Pulse Frequency 100 Hz 60 Hz Pulse Width 250 us 250 us Pulse Duration 500 us 500 us Maximum Intensity 16 mA 16 mA Ramp Up Time 14 minutes 14 minutes Steady Time 46 minutes 6 minutes Ramp Down Time 45 seconds 45 seconds Session Duration 60 minutes 20 minutes 2.1 DELIVERABLES Delivery of the Neuromodulation therapy devices and accessories will be completed on the 1st and 15th of each month. 2.2 SCHEDULE: The supplier shall deliver all items ordered under the contract during standard operating hours, Monday through Friday, excluding Federal Holidays, to the following location: Place Of Performance: SHIP TO: CPU BONHAM VA MEDICAL CENTER ATTN: SHELLMAN /VISN 17 1201 E. 9TH STREET BLDG 10 BONHAM, TX 75418 CPU Replenishment: Responsibilities: The contractor shall provide a monthly drop shipment to the CPU monthly. The contractor shall ensure appropriate stock is available to ensure prompt delivery can be made by the 15th of each month. CPU points of contact shall communicate with the contractor any issues concerning the shipment and delivery of products. Order Placement: The contractor shall deliver the requested quantities each month.
