Background: Compliance with Patient Safety Alert 24-01 requires replacement of 40 behavioral health mattresses currently used on MHRRTP unit. Performance Period/Schedule: The contractor shall deliver the requested items within 6 weeks of the date of award. Type of Contract: Firm-fixed-price General Item Requirements The contractor shall provide behavioral health mattresses with the following characteristics: Behavioral Health flat surface mattress Made in the USA 60 x80 x36 Welded Seams over a foam mattress Core and cover are fire retardant Additional Required Features of the Cover: Ripstop nylon Puncture resistant Fluid resistant Crack resistant Abrasion resistant Bedbug resistant Anti-microbial Anti-fungal Virus barrier Ability to be cleaned with hospital grade cleaners Breathable vent that repels liquids Patch kit available Schedule for Deliverables: Delivery will be made to the SVAHCS warehouse, Building 7. A loading dock is available, so a liftgate and inside delivery is not required. If for any reason the items cannot be delivered within the scheduled time frame, the contractor is required to explain why in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response, in accordance with applicable regulations. Changes to Statement of work: Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept by the CO. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
