    Description

    Amendment 02. This purpose of this amendment is as follows: 

    1) The offer due date has been extended to 22 May 2025 at 10:00 am PST.

    2) Attachment 2 (Instructions to Offerors) and Attachment 3 (Evaluation Criteria) have been removed. FAR 52.212-1(Instructions to Offerors) and a revised version of FAR 52.212-2 (Evaluation) are now included directly in the RFQ.

    3) The period of performance is updated to begin on 23 June 2025 and end no later than 26 August 2025.

    4) Attachment 4, the Statement of Need (SON), has been updated to include pictures of the affected rooms.

    5) Attachment 7, JA Building Layout, has been removed. The layout information has been consolidated into the SON, reflecting the affected rooms.

    6) Attachment 5, Questions and Answers (Q&As), has been uploaded for review.

    7) Attachment 8, Site Visit Information, and Attachment 9, EAL Template, have been removed. 

    8) Attachment numbering has been updated to reflect the removals and additions.

    Amendment 01. This amendment revises the Statement of Need (SON) criteria, specifically modifying the "cove base material" section. The change involves reducing the required cove base material from six (6) inches to four (4) inches.

    This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

    (ii) The solicitation number is FA468625Q0018 and the solicitation is issued as a request for quotation (RFQ).

    (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circulation 2025-03.

    (iv) This requirement will be set-aside as a Service Disabled Veteran owned Small Business Concern and the associated NAICS code is 238990 - all other specialty trade contractors and a small business size standard of $19M. 

    (v) A list of line item number(s) and items, quantities, and units of measure is identified in Attachment 1, Section B - Supplies or Services & Prices or Costs. Offerors shall return completed as part of quote submission.

    (vi) Description of requirement: The Contractor shall furnish all supervision, labor, tools, equipment, materials, operations, and incidentals as required to complete the following work: Carpet Removal and Installation. The contractor shall replace carpet with new carpet tiles and cove base in the Command Section and three adjacent offices (approx. 1530 sq ft), excluding tile areas. The contractor must follow the Air Force Carpet II Ordering Guide for material procurement (no markup allowed) and is responsible for moving and returning all furniture within the legal office as detailed in the SON. In addition to carpet replacement, the project includes ACM removal in the same area. The contractor must prepare the subfloor (grinding may be necessary), submit an abatement plan with certifications, remove/dispose of all ACM and debris per the plan, provide air clearance sample results via a CA-certified asbestos consultant, and submit NESHAP notifications to the EPA and base environmental contacts at least 10 business days before starting abatement. A hazardous waste manifest must also be submitted to the base environmental manager. The government will remove wall hangings, and the contractor will protect fixed items during ACM removal.

    (vii) Date(s) and place(s) of delivery and acceptable and FOB point is identified in Attachment 1, Section F - Deliveries or Performance Schedules.

    (ix) The provision at 52.212-2, Evaluation - Commercial Products and Commercial Services, applies to this acquisition and the specific evaluation criteria to be included is identified in Attachment 3. 

    (x) Offerors shall included a completed copy of the provision at 5.212-3, Offeror Representations and Certifications - Commercial Products and Commercial Services, with its offer.

    (xi) The clause at 52.212-4, Contract Terms and Conditions- Commercial Products and Commercial Services, applies to this acquisition

    (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders- Commercial Products and Commercial Services, applies to this acquisition and the additional FAR clauses cited in Attachment 1 are applicable to the acquisition.

    (xiii) There are no additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements) necessary for this acquisition

    (xiv) The Defense Priorities and Allocation Systems (DPAS) and assigned rating is not applicable.

    (xv) Offers are due 19 May 2025 at 10:00 AM PST to the POCs identified in the Contact Information Section of this notice.
