DeBakey VA Medical Center (MEDVAMC) Network Contracting Office (NCO) 16 intends to issue and award a Sole Source single award Firm-Fixed Price type contract with a performance period of one (1) year plus two (2) option years to BIOSENSE WEBSTER, INC. located at 31 TECHNOLOGY DR STE 200 IRVINE CA 92618, for CARTO 3 Electro-Anatomical Mapping System Service Plan (Hardware/Software/Warranty) at the MEDVAMC, located at 2002 Holcombe Boulevard, Houston, TX 77030. Based on our initial market research and due to proprietary concerns, we have found that only one reputable source can provide the CARTO 3 Electro-Anatomical Mapping System Service Plan for listed equipment in attached draft Statement of Work (SOW) below. The contract will be a Firm Fixed Price single award contract. The statutory authority for applying the Simplified Procedures for Commercial Items of FAR 13.5 is 41 U.S.C. § 1901 and is implemented by FAR 13.106-1(b)(2) for restricting competition on this procurement. Competition is restricted on this procurement for only one responsible source, and no other supplies or services will satisfy agency requirements. The North American Industrial Classification System (NAICS) code for this procurement is 811210, with a small business size standard $34.0M, and the FSC/PSC is J065. The Department of Veterans Affairs intends to solicit and award IAW FAR 13 a sole source firm fixed award to provide CARTO 3 Electro-Anatomical Mapping System Service Plan (Hardware/Software/Warranty) as list for the equipment identified in attached draft SOW.
