Loading dock area is limited space Is there room to park a semi truck at the purge location? If not, can we park a 28 foot Box truck? 5. Raised dock available If no raised or flat dock, how far away do we have to park from the file room? 6. File room door is less than 40 inches What is the width of file room door? Are there any smaller doors between the file room and the loading dock/loading area? 7. Pallet will fit all the way to the room but not inside. Can we fit a 40 x 48 inch pallet through the hallways/elevators leading to the loading dock? (we must be able to move a pallet and/or film cart through each door between the file room and the loading dock/loading area) 8. Describe the room where the film is stored. (i.e. rolling racks, fixed shelves, narrow aisles, very little space for purged materials, etc.) 16. Unknown, and not relevant to destruction. How are the patient files stored? (i.e. sorted by year, all years mixed together, alphabetically by patient name, MRI Number, etc.) 17. Unsure of the state of files. Is the file room flooded, and is there any water damage to the materials we will be destroying? 18. There was a site walk on 09/03/25 at 9am to view the materials, there is not a determined volume or poundage. Can you confirm how much needs to be destroyed, like poundage? 19. Price to remove all material, not on poundage or volume. do you just want us to price it per pound? 20. No known flood Was there a flood in the building or what happened to the floor? 21. Building two Basement Level. Same level as the loading dock. What floor are the records on? 22. Photos are for reference only, and not a measure of volume/weight/size of materials. Is everything that needs to be destroyed in the photos provided? 23. Normal business hours 7am-4:30pm. What hours will be have access to the building? 24. Is there limited access to the room where it's located? If it is on a ground level, a basement, or two or more floors. Is there an elevator? And how far from the elevator is the film located? The distance between the room and the elevator (if any), and the distance from the room/elevator to where we can park a 26ft truck? 26. Invoice at completion, and receipt of destruction documents. Would you allow the contractor to submit an invoice during the X ray film / medical records disposal project? This would help with operational expenses.
