Standard Form 18 including VI. Pricing Section

Signed copy of solicitation amendments (if any)

For the proposals over $30,000, proof showing that System Award Management Registry, Unique Entity ID and NCAGE numbers were completed.
Filled and signed NDAA 889 Compliance document in Attachment 1.
Filled and signed Certification regarding compliance with applicable federal anti-discrimination law in Attachment 2.
