    All offerors shall provide the following items:
    Please complete the SF1449: Blocks 17 (to include the CAGE code and the Unique Entity ID Number) and 30 A - C.


    Please provide a statement that the offeror will hold its quote firm for at least 90 days from thedue date for receipt of quotes, subject to any changes that may occur as a result of discussions (if applicable).


    Please complete the provisions, as listed in the solicitation. Alternately, complete all provisionsin System for Award Management (SAM) and provide confirmation of completion and accuracywith offer. Potential contractors must be registered in SAM to be eligible for award. The SAM website is http://www.sam.gov.


    Quotes are due by no later than (NLT) 11:00 AM/CT on 2 June 2026. Quotes shall be submitted via email to patrick.a.thielemann.civ@health.mil and salameya.paulouskaya2.civ@health.mil. Quotes received after the deadline will be considered untimely and will not be eligible for award.

    Questions are by NLT 11:00 AM/CT on 20 May 2026. All questions must be submitted in writing to patrick.a.thielemann.civ@health.mil and salameya.paulouskaya2.civ@health.mil. Electronic submissions with files larger than 20MB will need to be submitted in separate emails. All emails, regardless of the size, need to be received by the Government server, by the stated date and time. It is the offeror's responsibility to ensure the Government receives the entire quote.

    Factor 1. Technical Capability: The technical proposal shall be addressed in sufficient written detail for the Government to determine if the Offeror understands the technical aspects of the Government's requirement. Offerors shall demonstrate how they will comply with the Performance Work Statement (PWS) via the submission of a written capability statement, product literature, or other materials.


    Factor 2. Past Performance: The Offeror shall submit with its proposal evidence of recent and relevant past performance under which YOUR company (the Offeror) provided the same or similar services in the public and/or private sector. Performance must have been by the Offeror (not by a subsidiary or sister company) either as the prime contractor or subcontractor.
    Offerors shall complete and provide the Project Summary Sheet (Attachment 1) on three (3) task orders or stand-alone contracts/purchase orders the Offeror will complete or has completed within the past three (3) years from the closing date of this solicitation that the Offeror considers most relevant in demonstrating YOUR company's (the Offeror's) ability to perform the
    proposed effort. Past Performance must be with the Department of the Army, other US Government departments or agencies, state and local governments and/or commercial/private sector clients. If the Offeror performed as a subcontractor on the reference project, the Project Summary Sheet shall explain what percentage and type of work that was performed by the Offeror as subcontractor. In the instance that an offeror submits more than three (3) Project Summary Sheets, only the first three (3) will be evaluated. Offerors shall use the attachment provided.
    Past Performance Questionnaires (PPQs) (Attachment 2) may be submitted (however not required) for each past performance reference identified on the project summary sheet (Attachment 1).
    Contractor Performance Assessment Reports (CPARs) will also be accepted. For projects that have CPARs, Offerors can submit CPARs with Project Summary Sheets in lieu of PPQs. Offerors shall ensure CPARs are finalized and reference the exact same projects detailed in the Project Summary Sheet.
    If a PPQ or CPARs was not received for any of the past performance references identified on the project summary sheet, the Government may conduct interviews to validate performance/quality information. A past performance reference will not be evaluated if it does not contain valid/current POC contact information, or if the POC provided cannot validate work identified on the project summary sheet. The Government reserves the right to use both the information provided in the Offeror's Past Performance submittals and information obtained from other sources available to the Government, to include, but not limited to, the CPARs, Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or
    other databases; interviews with Program Managers, Contracting Officers, and commercial sources and/or non-Government sources, such as any commercial business or customers referenced by the Offeror in any of the past performance information disclosed.


    Factor 3. Price: Offeror shall complete the pricing for each of the contract line items (CLINS) under the Continuation of Supplies or Services and Prices/Costs or provide a separate document with the pricing for each CLIN.
