Attachment five (5) needs to be filled in as directed with YES or NO boxes checked and initialed and dated at the bottom. Attachment five (5) shall be returned with the filled in RFQ. This Award is Sole-Sourced and Patent Protected (attachment 1). Your quoted price must be all inclusive. This shall include the price for shipping, completing test reports, completing the Certificate of Compliance, and any other costs.
