Bidder must be licensed in all 50 states. Bidder must comply with all Utah State PBM legislation. Bidder must be able to provide a complete bid no later than February 13, 2026 Bidder must be able to integrate PBM services with current TPA, Public Employees Health Plan (PEHP) no later than August 1 (May 1 Kick off) and implement with JSD no later than September 1, 2026. Bidder must be prepared to offer both a 1 year and 3-year contract for JSD A 3-year contract must include a market check at18 months. A 3-year contract must include a termination for convenience clause Bidder must provide the following with their bid: Pricing commitment offers Contract Template Specialty drug list/limited distribution drug list at NDC level (excel format) Performance guarantees (implementation and ongoing) Formularies (by NDC in excel) Exclusion lists for low value, high-cost drugs Manufacturer drug coupon list Clinical program and menu with feesIf you are able to meet the minimum requirements to bid, please email Suzanne.Dolbey@alliant.com certifying your ability to meet these requirements and the appropriate claims and RFP questionnaire will be released to you via email.Please note that there is a 1GB limit on a single file upload.
