INSTRUCTIONS: (1) Under Supporting Documentation: Download the Vendor Application Form. The form must be downloaded and completed outside of the Bonfire platform. (2) Complete the Vendor Application and sign all Required Forms. You must provide a signature where required. Missing, incomplete or unsigned forms will result in vendor being eliminated. (3) Upload the completed Vendor Application under Submission by clicking the Begin Submission button.NOTE: YOU WILL NOT BE ABLE TO SUBMIT YOUR APPLICATION UNTIL YOU REGISTER AND CREATE AN ACCOUNT IN BONFIRE.
