    Evaluation Criteria

    • Experience of the Firm (275 pts)

      Describe the experience and qualifications of the firm in providing services for similar projects. Identify projects the submitting firm has completed in the last three years. For each project listed, provide:

      1. Description of the project including scope and project owner
      2. Role of the firm and explain how this relates to the services being solicited
      3. Project’s original contract value, final contract value, and reason for variance
      4. Project’s start date and completion date
    • Experience of the Key Personnel (275 pts)

      Describe the experience and qualifications of the specific project team expected to be assigned to the services proposed. For each key person identified, list their length of time with the firm. List each key person’s role in the projects provided. If a project selected for a key person is the same as one selected for the firm, provide just the project name and the role of the key person. For each project listed, provide:

      As part of our selection process, the City has the responsibility of considering the possibility of the firm receiving multiple task assignments under this contract overlapping the same time period, in addition to any other on-going work the firm may have.       2. Providing expedited services on requests for proposals and deliverables
      3. Providing quick turn-around times to inquiries

In order to be considered for this project, the contractor must hold the correct license as deemed appropriate by the Arizona Registrar of Contractors prior to submitting a Statement of Qualifications (SOQ) for this project in accordance with Arizona Revised Statute §32-1151.         2. Identify the location of the firm’s principal office and the home office location of key staff on this project.  **Ask PM if item 2.b is allowed for federal projects**
        3. Identify any contract or subcontract held by the firm or officers of the firm, which has been terminated within the last five years. Describe how you intend to implement this subcontractor selection plan including your recommendations for subcontractor trades to be selected by qualifications only vs. qualifications and bids; and discuss the benefit that your subcontractor selection plan provides to the project.
      3. Describe your team’s project management approach and team organization during design and construction phase services. Describe systems used for planning, scheduling, estimating, and managing construction. Briefly describe the firm’s experience on quality control, dispute resolution, and safety management.

The projects listed should show the breadth of the firm's experience and demonstrated capabilities and show the experience in managing multiple trade subcontractors, vendors, and suppliers. For each comparable project identified, provide:

        2. Role of the firm either as prime or subcontractor

    • Experience of Key Personnel to be Assigned (250 pts)

    Q (No subject): The 'Reference Check Instructions, Form and Sample Letter' has a number of items in red text including the due date for the Performance Evaluation form, the contract specialist name and phone number. Are we to assume that the due date for these reference checks is the same as the due date for the SOQ (April 10, 2026)?

    Q (Information Sheet): On page 6 of the QVL, it states not to provide any other detail other than the OPENGOV ID, Firm name, address, vendor ID, phone number, person name/title, email and phone number with no other information. On page 2, it requires the classification responding to be added to the information sheet. I assume these classifications being submitted be added to the information sheet. Please advise.

    A: Yes, that is correct, add the classifications for which the firm is qualified and interested in on the information sheet.


    Q (Consultant & Contractor Reference Check): The attachment has instructions, cover sheet and the actual reference form to be completed. What part is required back to the email address? Just the completed reference check form with project number/name, etc. No cover page is required?

    Q (QVL Cover Sheet vs. Information Sheet): Could you please define the difference between the "Cover Sheet" and the "Information Sheet" and the content of each. I assume the cover sheet is to display the firm name, vendor number, project title and project title as noted within the RFQ page 5? On page 6 of the RFQ, the Information sheet asks for a lot of the same information. Do the classifications requesting for the company go on the Information sheet as well?

    A: Firms typically place a cover page displaying just firms legal name, vendor number, project title and project number. Information sheet asks for more information such firm address, phone number, firms contact information such as email address of the person for the project.
