    Description

    Cobb & Douglas Public Health (CDPH) is seeking proposals from academic institutions and businesses to support the completion of a comprehensive Community Health Assessment (CHA). This assessment will follow the Mobilizing for Action through Planning and Partnerships (MAPP) 2.0 framework established by the National Association of County and City Health Officials (NACCHO). The selected contractor(s) will collaborate with CDPH to conduct key components of the required MAPP 2.0 assessments by providing data collection, analysis, and stakeholder engagement support. The resulting CHA will inform community health improvement planning and guide CDPH’s ongoing efforts to enhance the health and well-being of residents in Cobb and Douglas counties through the community health improvement coalitions, Cobb2020 and Live Healthy Douglas. It will also be used by Cobb & Douglas Public Health for Public Health Accreditation Board accreditation. This RFI will be used to facilitate the procurement of contractors to support components of the CHA and will inform the budget for fiscal year 2027.
