Package A: Annual Physical Exams & Related Services
The City is seeking proposals from qualified contractors to provide annual physical exams and related services to Fire Department personnel, in accordance with:
Proposers should have in-depth knowledge of fire service specific medical assessments, including baseline medical evaluations, consolidated medical assessment, immunizations (as needed), data record-keeping services, and including, but not limited to, electronic data reporting in accordance with the above-mentioned initiative, standard and programs. The City is requesting all physical examinations to be completed onsite at the a City facility.

Package B: Medical Advisor for Emergency Medical Services
The City is seeking proposals from qualified contractors to serve as a Medical Advisor to the Fire Department's Emergency Medical Services (EMS) program. The selected physician will be expected to provide prescriptive authority and oversight, protocol review and approval, case reviews, quality improvement activities, curriculum development and training and consultation on staff performance.
The primary physical proposed by Contractor to serve as Medical Advisor must meet the City's minimum qualifications. Failure to meet the minimum qualifications shall result into the proposer's automatic disqualification from this RFP without further consideration.
