FITNESS FOR DUTY & PEACE OFFICER PSYCHOLOGICAL EVALUATIONS

The County of Shasta, through its Department of Support Services, Purchasing Unit, invites responses to a Request for Quotes (RFQ) to provide Fitness for Duty (FFD) and Peace Officer FFD psychological evaluation services.

Background

The County of Shasta uses outside consultants to conduct psychological evaluations of prospective and current employees for the following purposes: hiring, employment related psychological evaluations, FFD evaluations, Peace Officer FFD evaluations, and other specialized psychological evaluations as needed.

Evaluation Criteria

• Overall experience/ability to perform Fitness for Duty and POST evaluations. (30 pts)
• Capabilities as it relates to in-person and tele-evaluations, schedule and findings report. (25 pts)
• The price identified for the service(s). (25 pts)
• Staffing availability in the provision of services. (10 pts)
• Proof of current licensure and completeness of Response. (10 pts)

Submission Requirements

• Provide a narrative describing experience and ability to perform Fitness for Duty psychological testing. (required)
• Provide a narrative describing experience and ability to perform pre-employment psychological testing/examinations. (required)
• Provide a narrative describing experience and ability to perform Peace Officer Standards and Training (POST) psychological suitability examinations. (required) —

By submitting a Proposal, the Respondent agrees to save, defend, keep, hold harmless, and fully indemnify County, its elected officials, officers, employees, agents and volunteers from all damages, claims, costs, or expenses, whether in law or in equity, that may at any time arise for not disclosing any portion of a Proposal marked “Confidential” to a third party.

• Contractor shall provide Performance Bond for each project totaling more than $25,000. (required)

• Contractor shall provide and pay for all labor, materials, equipment, tools, construction equipment and machinery, water, heat, utilities, transportation, and other facilities and services necessary for the proper execution and completion of the work. (required)

• Contractor at all times shall keep the premises free from accumulation of waste materials or rubbish caused by Contractor's operations. Upon completion of the work, Contractor shall remove all of Contractor's waste materials and rubbish from and about the project as well as all of Contractor's tools, construction equipment, machinery, and surplus materials. • Contractor will acknowledge that Contractor has made an independent investigation of the job site, including underground conditions and all other conditions that might affect the progress of the work. (required)
• Contractor acknowledges awareness of the provisions of subdivision (b) of Public Contract Code section 7103.5, which states as follows: (required) —
