    Please see attached scope of work and general requirements in the documents tab. *Note contractor is to provide and supply a 1-year warranty on material and labor. * Dear Vendor: Enclosed is a request for quotation (RFQ) to preform the attached scope of work. Mandatory site visit will be held either Wednesday, May 27th, 2026, 10:00 am or Friday, May 29th, 2026, 1:30 pm at the above address. Please confirm your attendance prior to the site visit by contacting Kevin Miller, MSIII (845) 416-5959 or via email at Kevin.x.miller@opwdd.ny.gov. Attendance will be recorded. Please be sure to sign the sign-in sheet (provided by the Maintenance staff conducting the site visit). The sign in sheet is a separate entity from the resident visitor’s log. When signing in, be sure to check the project name listed on the sign in sheet to ensure you are recording your attendance correctly. Failure to do so will cause out to be ineligible to submit a quote. There are no exceptions. Please write clearly for corresponding purposes and make sure you include an email address and phone number where you can be reached. Late arrivers at the site will not be allowed to sign in, participate, nor submit a quote for this project. If you are unable to attend either day, you will not be able to schedule a site visit at a different time. Method of Award: This is an RFQ (request for quotes). This is not an IFB (bid). The Hudson Valley DDSO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate 2 years of relevant experience on prior projects of similar magnitude and its subject to vendor responsibility determination. Hudson Valley DDSO reserves the right to reject any and all quotes. In deciding whether to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker’s Compensation Insurance as well as completed Lobbying Law Forms, and signed Appendix A. Please be prepared to submit these in a timely fashion. The Hudson Valley DDSO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition. The Hudson Valley DDSO’s interpretation of specifications shall be final and binding. Please note, all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitor log and/or an attendance sheet provided by the Maintenance Department. In the event of a tie, contract will be awarded to the Contractor whose bid was received earliest. Prevailing Wages: Schedule PRC #: 2026012320 NYS Labor law requires that all contracted employees performing maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or a building service contract covered by Article 9 thereof, neither Contractor's employees nor the employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law and as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Furthermore, contractors must pay at least the prevailing wage rate or provide the prevailing supplements, including the premium rates for overtime pay, as determined by the State Labor Department in accordance with the Labor Law. NYS law requires the vendor to keep detailed records for each person employed including name, address, telephone number, social security number, occupational classification in which worked, hourly wage rate paid, supplements provided, number of hours worked, and actual wages paid. Certified payroll record/s MUST be submitted at the time of invoicing. Effective April 28, 2008, if this is a public work contract covered by Article 8 of the Labor Law, the contractor understands and agrees that the filing of payrolls in a manner consistent with Subdivision 3-a of Section 220 of the Labor Law shall be a condition precedent to payment by the State of any State approved sums due and owing for work done upon the project. Without submitting certified payroll record/s, payment will not be made. Information on prevailing wage can be found at: www.labor.ny.gov *If you need any assistance understanding prevailing wages, please contact NYS DOL at (212) 932-2419* NYS Labor Law 220-i Registration Required: When submitting a bid to the Office for People With Developmental Disabilities (OPWDD) for Article 8 public work contracts, bidders must provide their NYSDOL registration number in the designated space on the bid form and submit their Certificate of Registration. Failure to provide a valid and verifiable registration number from the NYSDOL Registry and Certificate of Registration at the time of bid submission will result in disqualification of the bid. Subcontracting: OPWDD/Hudson Valley DDSO does not allow subcontracting on this type of work. Submission of quote: All quotes (including a breakdown of materials, labor, and overhead) MUST be received in the Hudson Valley DDSO Business office by 12:00 PM, Thursday, June 4th, 2026. The quotes may be hand delivered, mailed and or emailed as long as they are RECEIVED in the Business Office prior to the deadline. EMAIL IS PREFERRED! Please check your submission for arithmetic errors and correct labor wages. Hudson Valley DDSO reserves the right to reject any quote which does not reflect the correct prevailing wages in accordance with the wage schedule provided.
