Health Consultant Services / Scope of WorkThe type and frequency of consultation services shall be determined through mutual agreement between the Head Start/Early Head Start Director (or designee) and the Health Consultant. Records reviewed may include, but are not limited to:-Confidential Medical Records-Immunization Records-Physical Examinations-Dental Examinations-IEPs-Medical Statements-Medication Orders and LabelsConsultation and TrainingAt the request of the agency, the Health Consultant may provide consultation and/or training to staff and families regarding health and safety procedures, including but not limited to:-Best practices-MAOF requirements-Head Start Performance Standards-California Community Care Licensing regulations-Other applicable regulatory agency requirements-Existing care plans and procedures-Medication administration-Asthma-Seizures-Universal (standard) precautions-Oral healthHealth Referrals and Follow-UpAs applicable to contracted services, the Health Consultant may respond to health referrals generated by program staff. Consultation activities and applicable follow-up documentation may be maintained in the agency’s designated reporting system, including ChildPlus.Consultation services related to health referrals may include, but are not limited to:Meeting and collaborating with families, as needed, to obtain additional health information and support planning for care at school.Conducting home visits, when necessary.Consulting with community medical providers to obtain additional health information and/or recommendations regarding the child’s care at school.Providing guidance and technical consultation to staff regarding individual child health concerns, including:-Development of Individualized Healthcare Plans (IHPs)-Development of 504 Accommodation Plans-Review of plans with staffParticipating in Multidisciplinary Team (MDT) meetings, as applicable to the child’s individual needs.Providing consultation regarding health services and related concerns to program staff and families, as requested.Health Milestones and TimelinesReports and documentation may be reviewed in accordance with applicable program compliance requirements. Applicable follow-up activities and related documentation may be maintained in ChildPlus under the corresponding event or referral.Applicable reports may include:3010 / 3065 – Referrals for medication and/or health-related accommodations3015 / 3035 – Health file review reports3065 – Individualized Health Care Plans (IHPs)Applicable health accommodations, documentation, and follow-up activities should align with program requirements and regulatory timelines.Written ReportsThe Health Consultant may provide periodic documentation related to contracted services, which may include:Service logsAccountability or consultation summariesDocumentation of consultation activitiesInvoices for services renderedDocumentation submitted for invoicing purposes should align with services provided during the applicable service period.
