    Description

    The web-based tool will support the screening of approximately 25,000 participants in the Developmental Disabilities Administration waiver and will be utilized by non-clinical staff to conduct the screening. Additionally, the case managers, nurses, and the participant’s team will have access to the tool or results to use for effective and efficient person-centered planning. The DDA regional offices and administrative headquarter team will use the tool for collecting data, program planning, and determining needed support. A secure web-based system that identifies risks before a crisis occurs The Tool will be used by non-clinical staff Primary users for screening are case managers Results utilized by the participant’s team, DDA administrative teams, and clinical supports Guide Appropriate service planning and resource management Support Current Systems The tool will be able to integrate with Long Term Services and Supports (LTSS) Maryland or other systems utilized by DDA The Developmental Disabilities Administration is currently using multiple web-based online systems to determine the participant’s risks and the level of support needed. The Health Risk Screening Tool is currently utilized by the community case managers to determine the level of risks to prevent morbidity and mortality. The Support Intensity Scale is also used for program planning and determining the level of support. The goal of the agency is to determine if there is one tool that can be used by nonclinical staff to do both.
