Aligned with the strategic goal of Sustainable Access, this initiative addresses persistent gaps in chronic disease management while strengthening care coordination, provider capacity, data-driven decision-making, and long-term system sustainability.Through a coordinated, systems-based approach, the initiative supports the integration of consumer-facing technology, enhanced care management, and advanced analytics to improve outcomes for rural residents with diabetes. The program emphasizescollaboration among providers, payers, and community partners and prioritizes solutions that can be sustained beyond the pilot period.Please refer to Appendix B for additional details/requirements for the Scope of Services.
