    JTA is seeking a reliable and experienced supplier to deliver a turnkey Vendor Managed Inventory (VMI) solution for antifreeze at its Myrtle Campus.       This addendum is to correct the titles in the Section V. Evaluation Phase of the document. Phase 2 has been correctly named "Business Compliance" and Phase 3 has been correctly name "Pricing Evaluation."

    Evaluation Criteria

    • Determination of Responsiveness (1 pts)
      • Experience:
        Demonstrate a minimum of three (3) years of experience providing Vendor Managed Inventory (VMI) services, including at least two (2) contracts of similar size and scope.
      • Business Registration and license:
        Be properly licensed and registered to conduct business in the State of Florida via Sunbiz.org. Each Bid must include a copy of the Bidder’s State of Florida Certificate of Status or Good Standing and a copy of all necessary licenses, registrations or certifications that are required to perform the Work.  In addition, all licenses required for Bidders whose businesses and professions are regulated by the Florida Department of Business and Professional Regulation must be active and current at the time the Bid is submitted. 
      • Compliance Requirements:
        Meet all applicable insurance requirements and submit all required representation and certifications found in Section 5 of the OpenGov Solicitation.

    Submission Requirements

    • Minimum Requirements (required)

      Bidders must be in good standing and authorized to transact business in the State of Florida; must not be listed in the System for Award Management (SAM) as an excluded party; and must demonstrate compliance with any other minimum requirements as stated in the Scope of Work and/or Instructions to Bidders.  Each Bid must include a copy of the Bidder’s State of Florida Certificate of Status or Good Standing and a copy of all necessary licenses, registrations or certifications that are required to perform the Work.   

      In addition, all licenses required for Bidders whose businesses and professions are regulated by the Florida Department of Business and Professional Regulation must be active and current at the time the Bid is submitted. Failure to demonstrate compliance may result in being deemed non-responsive.  

    • Legal Entity of Bidder (required)

      Bidders must be registered with the Florida Department of State to engage in business in the State of Florida. If the Bidder is a joint-venture, the Authority will treat the entity as a general partnership for all purposes, and the joint-venture must submit, in addition to the other submissions required herein, evidence of the authority of the individual(s) submitting the venture's Bid that such individual(s) is duly authorized to bind the venture make all of the required statements, assurances and certifications on behalf of the entity.

      Submitting a Bid is deemed to be a representation and warranty by the entity and individual submitting it that such entity has duly authorized the Bid, and the individual(s) signing such Bid has the authority to submit the Bid and make all of the required statements, assurances and certifications on behalf of the entity.

      The Bid Pricing Form shall clearly indicate the legal name, address, and telephone number of the Proposer and accurately identify the type of legal entity of the Bidder (i.e., corporation, LLC, partnership, joint venture, individual). The Bid Pricing Form shall be signed by an individual with the authority to bind the Bidder and shall clearly identify (by typing or printing) the name of the authorized representative of the Bidder.

      Each other entity submitting a Bid shall cause the Forms to be signed by its appropriately authorized senior executive officer, manager, or general partner.
