    Description

    Balsz Elementary School District #31 is seeking qualified contractors to submit Statements of Qualifications for Job Order Contracting (JOC) services across various trades. The District intends to utilize JOC as an efficient, flexible project delivery method to perform a wide range of construction services, including but not limited to repairs, maintenance, renovations, and minor capital improvements across District facilities. Funding sources may include Bond Funds and District Funds.

    It is anticipated that contracts will be awarded to multiple qualified firms. Work will be issued on a job order basis; therefore, specific project scopes, schedules, and budgets are not defined at this time and will be provided at the time of each job order.

    Balsz Elementary School District anticipates the need for various construction trades to support repair, maintenance, and minor construction projects on an as needed basis throughout the District. Trades may include, but are not limited to, drywall, painting, concrete, framing, tile, electrical, and plumbing. This will be an "as-needed" "when-needed" contract. Bonding is required for each and every project at or above $100,000.

    Q (PPVF Form): Hello, The RFQ requests PPVF Surveys but no form is provided in the RFQ and no deadline is given. Can you confirm the deadline for surveys to be returned to the District, and provide the form as well? Thank you!

    Q (General Conditions Unit Costs): The unit of measure for the general conditions pricing page is all in "per hour". Many of the items are not measured in hours. For example, temp electrical would be a generator rental best reflected by longer terms or temp fence which is measured in linear feet. Also, dust control is listed twice - we recommend having one of those as SWPP plan preparation. We recommend the following units: temp elec - week, temp water - week, temp toilet - month, trash service (dump fee) - each, trash rental - month, temp fence - foot, trailer setup - each, SWPPP preparation - each.

    A: The General Conditions pricing page shall remain as issued. The unit of measure is intentionally standardized for evaluation consistency across all responses and shall not be modified. This is a Request for Qualifications and not a project specific bid. Pricing is being collected for comparative and normalization purposes only and does not represent actual job order pricing, which will be developed at the time of each individual job order.


    Q (Redundancies): In the Vendor Questionnaire you request the same information in multiple places. Will you revise solicitation or do we need to provide the information repeatedly. Safety - 8.12 and 13.5 - upload safety plan, 14.7 describe safety plan. Bonding - 8.10 Copy of bid bond, 13.2, 14.4 and 14.10 - bonding capacity letter Insurance - 8.16 and 14.5 - upload certificate of insurance, 14.5 - statement of insurance

    A: The solicitation will not be revised. The information requested in multiple sections serves different evaluation and compliance purposes. Where the same subject matter appears, respondents shall provide the information in each location as requested. Failure to fully respond to all sections may result in the submission being deemed non responsive in accordance with A.A.C. R7-2-1024 and Uniform Instructions 4.2.A.


    Q (Performance Evaluation Survey): In 8.8 - References, you request our list of references and indicate we are to send them a Performance Evaluation Survey. Is there a document you would like us to use? Where will our references submit these surveys?

    A: The Performance Evaluation Survey form is provided in the Downloads section of the solicitation. Offerors are responsible for sending the survey to their listed references. Completed surveys shall be submitted as instructed within the solicitation, and the reference form itself is included in PDF format within the Vendor Questionnaire.


    Q (Fingerprints): In #4 of Vendor Questionnaire it says - "If required to provide services on Balsz Elementary School District, #31 property at least five (5) times during a month, contractor shall submit a full set of fingerprints to the school district in accordance with A.R.S. 15-512 of each person or employee who may provide such service." We do a lot of work in schools and all of our employees have DPS Fingerprint Clearance Cards. Will this be sufficient? Or will we need to go through the added time and expense of fingerprinting them again?

    A: Valid DPS Fingerprint Clearance Cards are acceptable for compliance with this requirement. It is the Contractor’s responsibility to ensure all personnel maintain current and valid fingerprint clearance in accordance with A.R.S. 15-512 and applicable District requirements at all times while performing services.


    Q (Performance Evaluation Survey): The "Past Performance References Attachment" document indicates that "Offeror is responsible for sending a Performance Evaluation Survey to each client listed below." I don't see the "Performance Evaluation Survey" document as an attachment withing the RFQ.

    Q (Sample Bid Bond Form): Section 6 / the required upload list references a $100,000 bid bond. Does BESD have a required or sample bid bond form for this solicitation. Because bid bonds are often stated as a percentage of the bid amount, please confirm whether BESD is requiring a flat $100,000 bid bond for the RFQ response, and whether there is a specific bond form that must be used. If there is a required form, please provide it or identify where it can be downloaded.

    A: A bid bond is not required for this RFQ response. Offerors shall provide a letter from their bonding company confirming the firm’s bonding capacity of at least $100,000 or greater. No specific bond form is required.


    Q (Survey Form): I am not locating the Performance Evaluation Survey form that is in the supposed to be included in the RFQ packet. Can you please tell me what page that form is on?

    A: Please see Question #1


    Q (Firm Experience): Section 11.12 references “representative projects,” but the questionnaire appears to provide response fields only for 11.12.1 and 11.12.2, including “Identify Project #1.” Please confirm whether BESD requires only one firm project example for this section, or whether offerors should submit additional representative project examples as part of the response.

    A: Offerors shall provide the project examples as structured within the Vendor Questionnaire. Sections 11.12.1 and 11.12.2 represent the required project submissions. Additional project examples are not required and will not be evaluated.


    Q (Performance Evaluation Survey): Can you please direct me where to find the Performance Evaluation Survey to be sent to our References for completion? I do not see a link to the Survey form under Section 11.8.10. References; only RFQ References.pdf (Past Performance Evaluation References list). Thank you!
