      Provide a general description of the company and/or team that is proposing to provide the services, including sub-consultants.  For each key person, provide the following information:

      1. Percentage of time that each person will be committed to the project
      2. Length of time with the firm
      3. Applicable professional registrations 
    • Firm's Capabilities - Relevant Technical Experience (30 pts)

      Management and organizational capabilities

      Quality and cost control procedures/policies

      Accountability of firm to avoid change orders

    • Past Performance (35 pts)

      Provide a list of similar projects on which the project team has experience within the past three years. For each reference project, please provide the following information:

      1. Description of the project, including project name and location
      2. Project owner and/or client information
      3. Role of the firm, including a description of the services provided
      4. Role of each key team member who worked on the project and the percentage of time spent by each key team member on said project
      5. Approximate dates services were provided
      6. Total Fees Paid
      7. Reference information (contact name, email and current telephone numbers per project)
    • Scheduling / Team Project Manager / Communication (15 pts)

      Provide a proposed project management, scheduling, and communication plan. Include the proposed project manager, backup contact, staff availability, approach to managing multiple task orders, response times, quality control process, and communication procedures with City staff.

      Vendor shall provide a summary containing the following items:

      An expression of the Consultant’s interest in the project and possession of adequate, dedicated and responsible resources (personnel) and commitment to providing services. 

      Indicate Consultants Area(s) of specialty. 

      Identify the location of the firm’s principal office and the home office location of key staff on this project.

      Resumes shall not be more than one (1) page each for all personnel proposed to perform the services herein.

    • Acknowledgement of Submittal (required)

      The Undersigned hereby declares, as submitter, that the only persons or parties interested in this RFQ as principals are those named herein; that no elected official or employee of the City is in any manner interested directly or indirectly in this RFQ or in the profits to be derived from the contract proposed to be taken, other than as permitted by law; that this submittal is made without any connection with any other person or persons making a separate proposal for the same purpose; that the submittal is in all respects fair and without collusion or fraud.

      Applicable professional licenses, registrations, certifications, and contractor licenses, must be furnished if required

      By submitting a SOQ, the firm/individual certifies:

      It shall not discriminate against any employee or applicant for employment in violation of Federal Executive Order 11246.

      It has not given, offered to give, nor intends to give at any time hereafter, any economic opportunity, future employment, gift, loan, gratuity, special discount, trip favor or service to a City employee, officer or agent in connection with the submitted SOQ.  Failure to sign the SOQ, or signing it with a false statement, shall void the submitted SOQ and any resulting Agreement.

      It (including the firm/individual’s employees, representatives, agents, lobbyists, attorneys, and subcontractors) will refrain, under penalty of disqualification, from direct or indirect contact for the purpose of influencing the selection or creating bias in the selection process with any person who may play a part in the selection process, including the Selection Committee, elected officials, the City Manager, Assistant City Managers, Department Heads, and other staff. 

      In addition to reviewing and understanding the submittal requirements, it has reviewed the attached Professional Services Agreement including the Scope of Work and any Exhibits.

      That he/she, who submits herewith to the City of Tolleson, Arizona, a Statement of Qualifications:

      That all statements of fact in such submittal are true;

      That said submittal was not made in the interest of or on behalf of any undisclosed person, partnership, company, association, organization or corporation;

      That said submittal has not, directly or indirectly by agreement, communication or conference with anyone attempted to induce action prejudicial to the interest of the City of Tolleson, Arizona, or of any submitter or anyone else interested in the proposed contract; and further,

      That prior to the public opening, said submitter:

      1. Did not directly or indirectly, induce or solicit anyone else to submit a false or sham submittal;
      2. Did not directly or indirectly collude, conspire, connive or agree with anyone else that said submitter or anyone else would submit a false or sham submittal, or that anyone should refrain from submitting or withdraw submittal;
