Describe the experience and qualifications of the firm in providing services for similar projects. Identify projects the submitting firm has completed in the last three years. For each project listed, provide:

Use the form provided in the Attachments section to obtain at least three references. It is preferred no more than one be a City of Phoenix project. If your firm has not completed prior projects with other agencies, you will not be penalized. It is recommended there be references outside the City of Phoenix.

Q (Subs' vendor ID required? ): Do we need to include our subs' vendor IDs in our submission?
A: No, only need to include prime firm's vendor number.
Q (Conflict of Interest?): Will we be conflicted out of future contracts for the airport?
A: No.
Q (No subject): Could you please confirm that the “Vendor Questionnaire” referenced at the end of the RFQ refers only to the upload location in the OpenGov portal for the Statement of Qualifications, rather than a standalone document?
A: Vendor Questionnaire is referring to the reference check that is attachment A, which is a separate document.
Q (No subject): Are you accepting or considering reference checks for subconsultants as well in the potential 21 points awarded for this section?
A: References are only for the prime firm.
Q (No subject): The RFQ notes a 10-point minimum font size. Could you please confirm whether this requirement applies to the organizational chart, or if a smaller font is acceptable for this graphic?
A: 10 point font on everything so it's legible to the panel.
Q (No subject): Could you please confirm whether “contract value” refers to the firm’s original and final contracted fees, or to the overall project value/construction cost?
A: Firm's original contract value/fee, final contract value/fee, and reason for variance.
Q (Condition of speakers to be reused): As the speakers in T3 have been in place for 10 years, have they recently been test to verify performance. If not, will existing speaker verification be part of this scope?
A: The speakers in T3 have only been in place for five years and are assumed to be in good condition and the only thing required is integration into the emergency notification system only. However, request that firm will check for Terminal 3 is to verify T3 speaker functionality. All of the head end equipment will be replaced with the exception of the cabling and the speakers. Contractor shall identify any defective speakers (e.g. blown speakers) through field verification and observations. Provide a report on the existing speakers, identifying any speakers that need to be replaced in T3. We will have an allowance to speakers that need to be replaced in Terminal 3 as we do believe there are a few bad speakers that will need to be replaced.
Q (Structural engineer): During the pre-submittal meeting there was a request to have a structural engineer be part of the design team. What is the envisioned role of the structural engineer?
A: The structural engineer is required as we will be putting in conduit and adding weight to the above ceiling area and the structural engineer just needs to understand the volume of pathways and approve for structural attachment for conduit and pathways in the ceiling areas.
