      2. Attachment 1 - Professional Consultant Certification Form to delineate each service your firm offers.
      3. Standard Form (SF) 330 –Part I and II, with all sections completed. SF-330 can be obtained from U. S. General Services Administration (GSA) website - http://gsa.gov/forms , then select Standard Form on the menu and go to the SF 330 titled Architectural - Engineering Qualifications.
      4. Signed copy of Pinellas County Small Business Enterprise (SBE) certificate issued by the Pinellas County Economic Development.
      5. Copy of licenses and certifications
      6. Copy of certificate of insurance showing minimum required insurance coverage per Attachment 2.

Failure to do so shall be just cause for forfeiture of the proposal guaranty.
