    Pursuant to Chapter 103D, Subchapter 4.5, HRS, and as allowed by HAR §3-122-16.31, the Department of Health, Office of Primary Care and Rural Health, requests interest for participating in the Medicare Rural Hospital Flexibility Program (Flex). The purpose of the Request for Information (RFI) is to select provider(s) for the federal grant. Funding is provided through a federal Flex grant and is designed to help critical access hospitals (CAHs) do any or all of the following: 1) engage in quality improvement and reporting activities; 2) engage in financial and operational improvement and reporting activities; and 3) engage in population health improvement and reporting activities. To be considered, interested parties must be in a rural designated area in Hawaii; have a CAH designation—or in the process of obtaining such designation—from the Centers for Medicare and Medicaid Services (CMS); and submit a completed application.
