    Description

    The Long Island State Veterans Home is soliciting proposals for a secure, cloud-based rehabilitation management software system to manage physical, occupational, speech therapy and audiology services for geriatric residents/registrants of the Home and should take into consideration all aspects of rehabilitation operating in a skilled nursing facility including all aspects of rehabilitation management. Vendor must have a minimum of 5 years of experience along with Healthcare or Hospitals/Nursing Homes experience. The solution must seamlessly integrate with the NetSmart MyUnity EMR, support billing and claims workflows, ensure MDS compliance, and include features such as automated documentation alerts. Prices must be included for all modules and should remain in effect for the entire contract term. The Home is anticipating a go-live date of March 31, 2027. A non-mandatory walk-thru is scheduled for Friday, August 14, 2026, meeting in the Home’s Nassau Conference Room (Enter through front entrance) at 11am. In addition to the on-site meeting, the Home is also holding a Microsoft Team Meeting at 11am (link to be provided).
