Stony Brook Medicine, located in Stony Brook, NY, is seeking Professional Services to provide design, engineering, and limited construction administration services for plumbing modifications in the Dialysis Suite located on the 4th floor of Stony Brook University Hospital (SBUH). Estimated time frame for design: Final construction documents are required within a mutually agreed-upon period of time following the issuance of a purchase order. This is a two-year contract. Consultant Minimum Qualifications 1. Minimum of five (5) years of experience completing projects on a hospital campus of similar size with associated rules and regulations. Work portfolio to be included in RFQ package. 2. Bidders must supply at least three (3) references (inclusive of current contact information) from similar clients that they have successfully completed in the last five years. 3. The project includes dedicated chemically compatible dialysis waste piping and reinstallation of the existing Reverse Osmosis (RO) water system, based on the attached preliminary engineering assessment titled “Stony Brook Medicine Dialysis Suite Plumbing and Wastewater Engineering Assessment” prepared by Hayduk Engineering, LLC (dated May 27, 2025). Services Requested Professional Services to provide design, engineering, and limited construction administration services for plumbing modifications in the Dialysis Suite located on the 4th floor of Stony Brook University Hospital (SBUH). 1. Introduction and Background Stony Brook Medicine, located in Stony Brook, NY, is seeking Professional Services to provide design, engineering, and limited construction administration services for the relocation of the Dialysis Suite from the 17th floor to the 4th floor. The project includes dedicated chemically compatible dialysis waste piping and reinstallation of the existing Reverse Osmosis (RO) water system, based on the attached preliminary engineering assessment titled “Stony Brook Medicine Dialysis Suite Plumbing and Wastewater Engineering Assessment” prepared by Hayduk Engineering, LLC (dated May 27, 2025). 2. Project Description (Plumbing Engineering) 1. Review all applicable scope, project area, and documents (including the attached Hayduk Engineering assessment) to incorporate into complete project plumbing engineering drawings and specifications. 2. Survey and assess the project area. 3. Provide the following key project components (quantities shown are general approximations and may vary slightly as the design progresses): • Removal of Existing Fixtures: Remove 3 hand sinks from the dialysis bays; remove 2 hopper sinks and replace them with new hand sinks; cut and cap 2 existing kitchen sinks. • Installation of New Fixtures: Install 2 new showers, sink, and eye wash station in the new RO room; install 2 new sinks. • Dialysis Waste System: Install 10 dialysis waste wall boxes (recessed wall-mounted fixtures); install new underground waste piping that is chemically compatible to convey dialysis waste to the hospital’s industrial waste line for discharge to Suffolk County Sewer District #21; incorporate gravity flow where feasible and specify chemically compatible pumps only where required. No central treatment system is required. • Domestic Water Systems: Design and specify cold and tempered domestic water supply lines to support the new and modified fixtures. • Reinstallation of RO Water System: Design the reinstallation of the existing RO water system (OSMO 23G RO System, RO Storage Tank, and Grundfos MQ 3-45 Booster Pumps) in the new RO room on the 4th floor across the hall from the dialysis suite. Assess the condition and remaining useful life of the existing disassembled RO equipment and provide written recommendations regarding continued use, repair, replacement, or upgrade of any components found to be near or at end-of-life. Design the RO water piping layout to connect the RO system to the dialysis suite. 4. Develop and provide biddable design and construction documents to implement project scope. 5. Provide limited construction administration services for the project (including up to 3–4 site visits for observation, response to RFIs, and preparation of AHJ sign-off documentation). 3. General Services: The following basic services are required for any Stony Brook Medicine (SBM) project. Project specific services will be identified when a design is required. A. Provide staffing and sub-consultants as necessary for the timely performance of the services required to meet the project requirements. B. Prepare the New York State Department of Health (DOH) Certificate of Need (CON) and participate in their site visit. The Architect and Engineer will assist in the plan of correction for the New York State (NYS) Department of Health survey, if required. C. Provide Pre-construction Risk Assessment (PCRA) consisting of Interim Life Safety Measures Plan (ILSM) and Infection Control Risk Assessment (ICRA). The goal of this plan is to ensure that the project construction does not compromise the health and safety of any of the building occupants. This plan should denote the required separation of staff, patients and other building occupants, essential patient support services and the required means of egress from the actual construction site. Provide the plan to SBM for submission to the DOH for approval to commence construction, if required. D. Prepare a complete Environmental Assessment Form (EAF) and perform the needed studies to serve as documentation for responses provided on the EAF. Studies will include, but not be limited to, a State Environmental Quality Review Act (SEQRA). E. Employ sustainable technologies as demanded by the following: Stony Brook Medicine is a member of the University’s Sustainability Task Force and committed to sustainable design principles. We manifest this in high performance designs for buildings that minimize energy consumption, atmospheric emission, solid wastes, liquid effluents, and maximum indoor environmental quality, and create healthy productive environments. The consultant is required to adhere to this policy by employing sustainable technologies in designs and construction. The successful consultant will be required to complete energy reduction documents as provided by SBM including baseline electrical/HVAC consumption and proposed energy conserving data for related disciplines and file with utilities for rebates. F. SBM has an ongoing sustainable program and must comply with the goal of NYS Executive Order 88 on all projects. Further, it is the policy of SBM that all projects must comply with United States Green Building Council (USGBC) LEED silver requirements. A LEED scorecard shall be prepared and provided to SBM for review. G. The consultant shall utilize Projectmates, SBM’s document retention program, to coordinate communications, schematic/construction drawings, shop drawings, submittals etc. SBM will provide educational sessions for Projectmates as necessary. Note: Design documents, related printed and electronic files become the sole property of Stony Brook Medicine. H. The consultant shall meet with SBM’s representatives to develop specific project requirements. Include a review of the issues relating to compliance with various accreditation and New York State Codes and Standards. I. K. SBM Policies and Procedures require that all people working on site must wear ID Badges. The consultant together with his sub-consultants shall obtain badges and pay the associated fee. L. The Consultant shall attend, together with its sub-consultants, all conferences required by SBM and take and distribute the minutes of all such meetings. M. Prepare specification manual to compliment biddable construction drawings using the 2020 CSI standard format. N. Provide all documents in electronic format. Construction drawings are to be in AutoCAD 2018 and specification in Microsoft Word 2016. Note: Design documents, related printed and electronic files become the sole property of Stony Brook Medicine. O. Assign a Construction Administrator (CA) to visit the construction site at agreed times during the construction phase to attend progress meetings, review all revisions, and field modification of the intended design, and issue updated construction reports and meeting minutes to SBM. P. Provide SBM two (2) sets of all Schematic, Design and Construction Documents, and post electronic copies in Projectmates. Q. Provide one (1) paper set of “As Built Drawings” and a set on AutoCAD to SBM’s architectural services (HAS) department that shall be loaded into Projectmates. 4. ENGINEERING AND DESIGN SERVICES A. DESIGN DOCUMENTS 1. Attend a kick-off meeting with any sub-consultants to review the scope of work, deliverables, project schedule, and procedures for work in Stony Brook Medicine (SBM). 2. Survey and collect field information required and meet with SBM personnel to understand the operation of the systems within the existing project location. 3. Allow as many site visits during the design and inspection phase as required to capture and implement the full scope. 4. Incorporate all recommendations in the Schematic Design and Basis of Design Report to finalize all equipment, material and routing. 5. Review of any of the existing documentation including, but not limited to, the drawings, in order to determine the requirements for additional information. 6. Review all codes, regulations and standards that may apply to this project. 7. Prepare and furnish Schematic Design and BOD Report (30% submission) consisting of design concept drawings showing the general layout, system description, sketches, recommendations, schedule, and rough cost estimate (construction) to SBM for review and approval. 8. Proceed with the Design Documents based on SBM review, comments and written approval of the Schematic Design and BOD Report. Incorporate all comments received and finalize all equipment, material and routing. 9. Prepare and furnish Design Documents (60% submission) that will address equipment, materials, and elements, including floor plans, and all other relevant data for SBM estimating to verify cost estimate to further refine and develop the scope. Further investigate General code, etc. along with the incorporation of all , phasing, architectural, interior design, structural, mechanical, electrical, plumbing, and fire protection system integration (if relevant to the plumbing scope). 10. Prepare and furnish specifications as required to detail and describe the work required, including finishes, quality of materials, and workmanship expected of the contractor. The specifications will include third party testing requirements and procedures, if applicable. 11. Review manufacturer’s submittals and present to SBM for approval during this Phase. Advise on coordination and lead times, if necessary. 12. Prepare and furnish an analysis of the probable cost of sufficient detail to indicate that construction of the proposed design is within the limitations of the project budget. 13. Prepare and furnish a project schedule for successful completion within an allowable timeline. 14. Prepare and submit to SBM for approval a Design Report consisting of system description, drawings, code review, list of specifications, long-lead equipment, permits, rough cost estimate, and project schedule. Delineate all assumptions and design calculations referenced. 15. Meet with SBM HAS and PMO to review the Design Documents. 16. The Engineer/Architect will coordinate all Consultants’ work respective to project scope. The Engineer/Architect will serve as the prime consultant responsible for monitoring the progress of, and leading the coordination of all other Consultants involved as applicable to support this scope. 17. Incorporate all addenda, and issue a complete set of Design Documents to SBM. Send PDF and electronic (AutoCAD) format copy to HAS. Note: Design documents, related printed and electronic files become the sole property of Stony Brook Medicine. B. CONSTRUCTION DOCUMENTS 1. Attend meetings with any sub-consultants to review the scope of work, deliverables, project schedule, and procedures for work at SBM. 2. Proceed with the Construction Documents based on SBM review, comments and written approval of the Design Documents. 3. Incorporate all comments received in the Design Documents phase and further refine all final equipment, material, and routing. 4. Prepare and furnish biddable drawings and specifications (100% submission) to select and define all materials and procedures of work. Construction documents must include a specification for third party testing (if applicable). 5. Refine the rough cost estimate and project schedule, based on the one hundred percent (100%) documents. 6. Meet with SBM HAS and PMO to review Construction Documents, cost estimate, and project schedule. If the project is over budget, the Consultant shall work with SBM to value engineer the design to bring it back within budget. 7. Incorporate all addenda and amendments, and issue a complete set of Construction Documents to SBM. Send PDF and electronic (AutoCAD) format copy to HAS. Note: Design documents, related printed and electronic files become the sole property of Stony Brook Medicine. C. BID REVIEW 1. Prepare (and if bidding is done), submit to SBM for approval a Pre-Bid Report and final Construction Bid Documents, which shall include, but not be limited to, final design drawings, specifications, final cost estimate, and project schedule, setting forth in detail the requirements for the construction of this project. 2. Assist SBM in obtaining competitive bids for the work. (If applicable) 3. Attend mandatory pre-bid conferences/walkthroughs, receive RFIs from the bidders, and issue all addenda as necessary. (If applicable) 4. Assist SBM in the evaluation of the bids. (If applicable) 5. Attend a pre-award conference with the successful bidders. (If applicable) 6. Advise SBM of any objections to the proposed list of contractors, sub-contractors, vendors, and material suppliers. D. CONSTRUCTION ADMINISTRATION (Limited per project scope — up to 3–4 site visits for observation, RFI responses, and AHJ sign-offs) 1. Schedule and conduct construction and progress meetings with the Contractor, the commissioning agent, and SBM to discuss such matters as, but not limited to, procedures, progress, shop drawings, scheduling, and RFI’s. Prepare and promptly distribute minutes to SBM and all stakeholders. 2. Attend a construction kick-off meeting with all parties to review the scope of work, schedule, and other contract matters pertaining to the project. 3. Provide equipment coordination and advise SBM on lead times. 4. Review each Contractor’s schedule of values and forward it to SBM with recommendations for approval. (if required) 5. Review and approve each Contractor’s CPM schedule. Check for compliance with the Contract Documents and forward them to SBM with comments. (if required) 6. SBM PMO shall coordinate third party testing and inspection independently and shall communicate information to the Contractor. 7. Receive, review, and respond to RFIs, shop drawings, project data, samples, and submittals required by the Contract Documents for compliance with the Contract. Transmittal and processing of all documents shall be through Projectmates. 8. Interpret and decide matters concerning performance under requirements of the Contract Documents on written request of either SBM or Contractor(s) through Projectmates. 9. Prepare and track all Design Change Notices (DCN) and issue them to SBM for review and approval through Projectmates. 10. Prepare and track all Change Orders (CO) and Construction Change Directives (CCD) through Projectmates. 11. Review each Contractor’s Application for Payment (AIA G702 and G703) and supporting documents shall be included with each application for payment. (if required) 12. Prepare a Project punch list to minimize deviations from scope/design. 13. Consultant shall provide commissioning assistance related to design, during start-up and testing of the system. Issue field reports and sign off on the final report through Projectmates. 14. Consultant shall review the contractor close out package of assembled written guarantees, manuals, and As-Built drawings to ensure the package is complete before transmitting to SBM. The consultant shall return the package to the contractor if incomplete. 15. Conduct inspections to determine the accuracy of the date(s) of substantial completion and the final completion. 16. A detailed index of the content of the proposal. 9. A statement of objectives and approach. Bidders should use this section to describe the proposed design, including any essential exhibits, and address all the issues listed in Statement of Work. SBUH reserves the right to reject any or all proposals, negotiate terms, or cancel the RFQ. All proposals become the property of SBUH. The selected firm must sign a standard SBUH professional services agreement and comply with all applicable laws, including non-discrimination and prevailing wage requirements. Stony Brook Medicine (SBM) is committed to following the Hospital's leadership mandate to implement sustainable and/or energy-conserving products/building materials that would be required to achieve a minimum of LEED Silver status. LEED certification may or may not be applied for but the required documentation should be appropriately available upon the request of SBM. Therefore, procured products shall meet the established sustainable standards consistent with their restrictive industries such as Green Guard, FloorScore for flooring products, BIFMA for Business and Institutional Furniture, CertiPUR-US for products containing foam material, etc. All vendor estimates, proposals, and quotes must be accompanied by the appropriate reference to the respective certifying agency and or the products within the design that meet these standards. N.Y. State Finance Law §139-l Pursuant to N.Y. State Finance Law §139-l, every bid made on or after January 1, 2019 to the State of any public department or agency thereof, where competitive bidding is required by statute, rule or regulation, for work or services performed or to be performed or goods sold or to be sold, and where otherwise required by such public department or agency, shall contain a certification that the bidder has and has implemented a written policy addressing sexual harassment prevention in the workplace and provides annual sexual harassment prevention training to all of its employees.
