    Evaluation Criteria

    • Experience of the Firm (275 pts)

      Describe the experience and qualifications of the firm in providing services for similar projects. Identify projects the submitting firm has completed in the last three years. For each project listed, provide:

      1. Description of the project including scope and project owner
      2. Role of the firm and explain how this relates to the services being solicited
      3. Project’s original contract value, final contract value, and reason for variance
      4. Project’s start date and completion date
    • Experience of the Key Personnel (275 pts)

      Describe the experience and qualifications of the specific project team expected to be assigned to the services proposed. For each key person identified, list their length of time with the firm. List each key person’s role in the projects provided. If a project selected for a key person is the same as one selected for the firm, provide just the project name and the role of the key person. For each project listed, provide:

      1. Description of the project including scope and project owner
      2. Role of the team or team member and explain how this relates to the services being solicited

        1. List the Arizona contractor licenses held by the firm and the key personnel who will be assigned to this project. Provide the contractor license number. Reference the appropriate licenses held, if needed. In order to be considered for this project, the contractor must hold the correct license as deemed appropriate by the Arizona Registrar of Contractors prior to submitting a Statement of Qualifications (SOQ) for this project in accordance with Arizona Revised Statute §32-1151. (PM to identify any specialized licensing requirements)
        2. Identify the location of the firm’s principal office and the home office location of key staff on this project.  **Ask PM if item 2.b is allowed for federal projects**
        3. Identify any contract or subcontract held by the firm or officers of the firm, which has been terminated within the last five years. Identify any claims arising from a contract which resulted in litigation or arbitration within the last three years. Briefly describe the circumstances and the outcomes.
    • Experience and Qualifications of the Team (250 pts)
      1. Identify at least three comparable projects in which the team served as either designer of record, general contractor, or member of a DB team. Special consideration will be given to teams that have led Design-Build teams on similar successful projects. For each project identified, provide the following:
        1. Description of the project
        2. Role of the firm (specify whether Design-Builder, Construction Manager or General Contractor. If Design-Builder, or General Contractor, identify the percent of work self-performed. Also specify services provided during design phase, i.e. cost estimating, scheduling, value engineering, etc.)
        3. Project’s original contracted construction cost and final construction cost.
        4. Construction dates
        5. Project owner
        6. Reference information (two current names with telephone numbers per project)
      2. List of all City of Phoenix projects where the team provided DB services, Construction Manager at Risk, agency construction management, or general construction services in the last five years, either completed or ongoing.

      2. Attached to this Request for Qualifications is the City of Phoenix general subcontractor selection plan (see Exhibit A). Describe how you intend to implement this subcontractor selection plan including your recommendations for subcontractor trades to be selected by qualifications only vs. qualifications and bids; and discuss the benefit that your subcontractor selection plan provides to the project.
      3. Describe your team’s project management approach and team organization during design and construction phase services. Describe systems used for planning, scheduling, estimating, and managing construction. Briefly describe the firm’s experience on quality control, dispute resolution, and safety management.

    • Experience and Qualifications of the Firm (250 pts)
      1. Identify at least three comparable projects in which the firm played a major role in the last three  years. The projects listed should show the breadth of the firm's experience and demonstrated capabilities and show the experience in managing multiple trade subcontractors, vendors, and suppliers. For each comparable project identified, provide:
        1. Description of project
        2. Role of the firm either as prime or subcontractor
        3. Project’s construction cost
        4. Construction dates (identify if this included phased construction)

    • Experience of Key Personnel to be Assigned (250 pts)
      1. For each key person identified, list their length of time with the firm and at least two comparable projects in which they have played a primary role in the last three years.       1. Describe the firm’s project management approach including its perspective and experience on partnering, quality control, project scheduling, claims, dispute resolution, changes in the scope of work, and construction safety.
      2. Describe software systems and processes used for planning, project engineering and scheduling, estimating, and managing construction.
      3. Provide a proposed subcontractor selection plan. The selection plan must select subcontractors based on qualifications alone or on a combination of qualifications and price and shall not select subcontractors based on price alone. Describe how you intend to implement this subcontractor selection plan. Discuss the benefit that your subcontractor selection plan provides to the project.
    • Construction Administration and Inspection Experience of the Prime Firm (175 pts)

      Describe the experience and qualifications of the prime firm in providing CA&I services for similar projects. Identify at least two projects the submitting firm has completed within the last five years. For each project listed, provide:

      1. Description of the project, including scope and project owner.
      2. Role of the firm and explain how this relates to the services being solicited. 
      3. Project’s original contract value, final contract value, and reason for variance.
      4. Project’s start date and completion date.
    • Construction Administration and Inspection Experience of Key Personnel and Subconsultants (325 pts)

      Describe the experience and qualifications of the project team, expected to be assigned to this project, in providing CA&I services for similar projects. For each key person identified, list their length of time with the firm. Explain the project team’s (including the construction inspector) knowledge and understanding of MAG Specification and Details, City of Phoenix Supplement to MAG Specification and Details, and City of Phoenix Standards as they relate to rehabilitation and maintenance of sanitary sewer infrastructure. Identify similar projects the key staff has completed within the last five years. If a project selected for a key person is the same as one selected for the firm, provide just the project name and the role of the key person. For each project listed, provide:

      1. Description of the project, including scope and project owner.
      2. Role of the firm and explain how this relates to the services being solicited.
      3. Project’s original contract value, final contract value, and reason for variance.
      4. Project’s start date and completion date.
    • Project Understanding and Approach (350 pts)

      Describe your firm’s understanding of the City’s need for this project, including important considerations such as project issues and challenges of various pipe sizes of sanitary sewer infrastructure. Explain the project team’s (including the construction inspector) knowledge and understanding of common sanitary sewer pipe maintenance and rehabilitation solutions. Explain the firm’s understanding and approach to emergency responses and planned construction. Describe the team’s approach to the project, including important considerations such as scope, schedule, and budget.

      Use the form provided in the Attachments section to obtain at least three references. It is preferred no more than one be a City of Phoenix project. If your firm has not completed prior projects with other agencies, you will not be penalized. It is recommended there be references outside the City of Phoenix.
