Supply all forms, posters, brochures, mailers, and cover all printing and mailing costs. The City reserves the right to review all printed materials prior to their distribution. All reports, enrollment materials, and marketing support must be provided at no cost to the City, including mailing costs associated with marketing efforts.5. If this RFP results in a new provider, any active employee enrolled in the corresponding Aflac product on the plan effective date will be allowed to enroll in the new provider’s comparable plan without Evidence of Insurability (EOI), with no new waiting periods or pre-existing condition limitations imposed for amounts up to their current in-force coverage (or the new provider’s guaranteed-issue maximum, if higher).6. Include coverage for the employee and their eligible dependents.7. Include coverage for major diseases/conditions, such as cancer, cardiovascular conditions, and infectious diseases.8. Not reduce coverage due to age and is portable for life without evidence of insurability.9. Handle all enrollment and claims processing.10. Offer an online platform for subscribers to access information on their plan benefits and cost of premiums, claims data, customer support, and enrollment information.11. Provide a dedicated implementation team to ensure a smooth transition if RFP results in the City selecting a new provider.12. Permit employees to enroll in the plan during their initial benefit eligibility window of thirty (30) daysfrom their date of hire or during the City’s annual open enrollment period.a. Provide access to secure on-demand information or reports to the City for the City to accurately perform a monthly data reconciliation of enrollment, payroll contributions, and premiums.24. Fully disclose all commissions, administrivia fees, overrides, marketing allowances, and any third-party payments.25. Provide consolidated electronic billing for all product lines.26. Work with the City to reconcile discrepancies in enrollment and payroll deduction reports.27. Receive the employee payroll contributions from the City via Automated Clearing House payments.
