Coatesville, PA 19320 Erie VA Medical Center 135 East 38th St. Erie, PA 16504 Lebanon VA Medical Center 1700 South Lincoln Ave Lebanon, PA 17042 Pittsburgh VA Healthcare System University Drive C Pittsburgh, PA 15240 H.J. Heinz Campus 1010 Delafield Road Pittsburgh, PA 15215 Philadelphia VA Medical Center 3900 Woodland Ave. Philadelphia, PA 19104 Wilkes-Barre VA Medical Center 1111 East End Blvd. Wilkes-Barre, PA 18711 Wilmington VA Medical Center 1601 Kirkwood Hwy Wilmington, DE 19805 The objective of this sole source award is for a firm fixed price contract with a three-year requirements contract with Bayer Healthcare, LLC. This sole source determination is in accordance with FAR 13, Simplified Acquisition, procedures and in accordance with 41 USC 253(c)(1) as implemented by FAR 6.302-1, only one responsible source and no other supplies or services will satisfy agency requirement. Cepheid is the only known authorized servicing agent for the Medrad systems at the locations provided above. The North American Industry Classification System (NAICS) code for this procurement is 811210. The Small Business Administration (SBA) size standard is $34 Million. The PSC code is J065, Maint/repair/rebuild Of Equipment- Medical, Dental, and Veterinary Equipment and Supplies. This notice of intent to sole source is not a request for quotes. Interested vendors that can meet this requirement (draft PWS is attached), should identify their interest and capability, and respond to this notice by 05/29/2021 by 3:00 PM EST. Interested vendors must send a capability statement, no more than one page in length; to include Company Name, UEI Number, Socioeconomic Category, brief description of your business entities capability to provide these services.
